EndlessWell welcomes both insurance and self-pay patients. Your cost may vary based on how your appointment is scheduled, your insurance plan, deductible, copay, coinsurance, and the services provided.
USING INSURANCE
Your options may look different depending on your plan.
EndlessWell participates with select insurance plans. Participation can vary by plan or product, even within the same insurance company.
Insurance benefits are verified electronically when available. Verification of benefits is not a guarantee of coverage or payment.
Your insurance plan determines your deductible, copay, coinsurance, allowed amounts, and final claim processing. You are responsible for understanding your benefits and for amounts assigned to you by your insurance plan.
Known patient-responsibility amounts are due before your appointment.
SELF-PAY & FEES
Straightforward self-pay pricing.
Self-pay is available for patients who are not using insurance for the visit, when permitted by applicable payer requirements, and for services that are not covered by a patient's plan.

Self-pay fees are due before the appointment. Additional testing or services may have a separate charge when clinically appropriate.
Insurance coverage for ADHD testing varies by plan. Testing services that are not covered by insurance may have a separate self-pay cost.

PRACTICE POLICIES
Payment
Payment is due before your appointment. Known copays, coinsurance, deductibles, self-pay fees, and other patient-responsibility balances must be paid before the visit unless another arrangement has been approved in advance. Patients can review and pay available balances through the EndlessWell Patient Portal.
Cancellations & Missed Appointments
Please give at least 24 hours' notice if you need to cancel or reschedule. Appointments canceled with less than 24 hours' notice is subject to a late-cancellation fee, and missed appointments are subject to a no-show fee, when permitted by applicable payer or program requirements.
